The History of Skin Needling
Dermal needling in the aesthetic arena is linked back to Andre Camirand who in 1995 discovered that dry needling using a tattoo gun without ink could improve the appearance of post- surgical scars.
By 2000 , Plastic Surgeon Dr Fernandez and German inventor Leibl teamed up to develop the dermal roller device consisting of a rolling cylinder of fine needles which when glided across the skin formed a series of small punctures in the dermis.
In the early days, Dr Fernandez clinical skin needling was quite invasive, often requiring a general anaesthetic or heavy sedation. The skin needling device was rolled vigorously over the patients skin to a depth between 1 and 2 mm, to a point where blood would ooze heavily to the skin's surface, coining the original use of the term "Vampire Lift" procedure.
This original technique certainly achieved results with a single treatment but the amount of downtime required for recovery was unacceptable to patients and the procedure began to fade.
With further research over the past two decades, studies have shown that similar results can be achieved with a series of less aggressive skin needling treatments rather than a single invasive procedure. The target collagen is within the upper 0.5 mm of the dermis and can be targeted with newer microneedling devices.
Rather than manual rolling of a device across the skin, newer technologies such as the Dermapen use an electronic device to deliver the micropunctures to the precise level in the skin and at a much higher density of microchannels per square centimetre of skin surface. Larger areas such as face, neck and chest can be covered more efficiently with minimal discomfort and less trauma to the upper epidermis. Although more treatments are required, this translates to effective treatment with less downtime.
What is Skin Needling and How Does Dermapen Work?
Dermapen, a microneedling pen, uses microneedle technology to kickstart a regenerative and healing cascade in the cells of the skin. This process is referred to as collagen induction therapy.
Using an oscillating array of 12 very fine needles, the lastest Dermapen 3 device is able to make 1300 microchannels in the skin per second which triggers the skin’s natural healing response. Watch this short video by Dermapen Australia to see how Dermapen works.
The stimulation of new collagen fibres leads to skin tightening and improvement in fine lines, wrinkles and scars. The fine microchannels in the skin also enable deeper absorption of active anti-aging skincare ingredients.
Clinical studies on skin microneedling reveal that dermapen treatments work by a process called neocollagenesis , that is, the formation of new collagen in the skin. Under the microscope, skin that has been treated with microneedling has an increased amount of collagen as well as a remodelling of existing collagen and a thickening of the top epidermal layer of skin. In aging skin this can be seen as an improved texture, clarity and glow to the skin and the improvement in skin elasticity makes dermapen a valuable anti-aging tool.
What Happens During a Dermapen Skin Needling Treatment?
At Cosmetique Solutions all potential candidates for Dermapen have a thorough consultation with Dr Kylie Regli before their treatment to ascertain the reason for treatment and to ensure they have no active skin conditions or infections which could preclude them from treatment.
Patients should allow at least an hour for the Dermapen procedure.Before the procedure and after the skin is cleaned of make-up and any residue, a compounded topical anaesthetic cream is applied to the face for 30 minutes in order to minimize discomfort during the procedure.
Once the numbing cream is removed, the skin is prepared with an antiseptic solution to ensure a sterile environment before the skin needling treatment is commenced.
The needle depth on the Dermapen is adjusted to allow for an individualised approach to skin rejuvenation depending on the patients skin type, nature of their concern and area to be treated. For example, needle depth is increased in areas of acne scarring and reduced in the more delicate under eye region.
A separate disposable sterile needle cartridge and sleeve is used for each patient to prevent cross contamination. A hyaluronic gel is coated on the skin to allow the vibrating needle array of the Dermapen to glide across the skin in three passes at different directions. This allows for maximum coverage of the skin surface a highest concentration of micropunctures in the skin. The more micropunctures , the more collagen production and remodelling!
At conclusion of the treatment a hyaluronic gel mask is applied to the skin for 10 minutes , followed by an optional Healite II LED phototherapy session to sooth the skin and promote faster healing.
Does Skin Needling Hurt?
Application of the prescription-only numbing cream available to Dr Kylie Regli, combined with the vibrating technology of the Dermapen 3 makes for a very tolerable procedure with minimal discomfort.
The idea of multiple needle pricks over the entire surface of the facial skin can be daunting to many people, however, the Dermapen treatment does not feel like needles at all. It is more of a tingling sensation as the Dermapen glides over the cool gel applied to the skin.
Dermapen Post Treatment
Directly after treatment, patients usually experience redness and very mild swelling. For the first 24 hours the skin takes on a mildly sunburnt appearance and may feel dry for a few days. Make-up can be worn the following day after treatment to camouflage any residual redness and the sun should be avoided for at least a week.
The Healite II LED phototherapy that we offer our patients at Cosmetique Solutions, directly after their microneedling procedure also speeds up the recovery time and further stimulates collagen production. It's the perfect synergy between two anti-aging therapies.
How Many Derma Needling Treatments are Needed and How Often?
For general rejuvenation to improve skin texture and help combat fine lines, at least 3 treatments 4 to 6 weeks apart are recommended. Acne scarring may need more treatments over a longer period of time. Scarring needs to be assessed on an individual basis depending on the type and depth of the scar tissue. That's why a consultation with Dr Kylie Regli is important prior to undergoing Dermapen treatment.
Many of my patients use Dermapen as a preventative skin aging regimen. Two to three Dermapen treatments per year will help the skin to naturally regenerate collagen and maintain elasticity. It's an insurance policy against aging skin, says Dr Kylie Regli.
Most people begin to see improvements in skin tone and texture within about 8 days after a Dermapen skin needling treatment, however the skin continues to improve over several weeks due to the nature of the skin's natural wound healing response and the induction of growth factors and collagen fibres.
